The girls will also get personal instructions from a professional hairstylist & makeup artist.

In today’s society, grace and poise are two things that young ladies are quickly losing. As they grow up in a world centered around social media, they are quickly losing the ability to enjoy a poised conversation, carry themselves in an attractive way, and be attentive to the needs and interests of those around them. This workshop provides the advice today’s young ladies need to grow into polished and poised young adults. Our Professional image staff will join us for a 30-minute presentation on taking care of your skin and natural-looking makeup tips, as well as a 30-minute presentation on the best quick and effortless ways to wear your hair based on your hair’s texture and facial shape.

General  Terms & Information
The Coaching relationship is unique in that it is a highly personal and at the same time, a contractual agreement. Given this, it is important for us to reach a clear understanding about how our relationship will work, and what each of us can expect. This consent will provide a clear framework for our work together. Feel free to discuss any of this with me. Please read and indicate that you have reviewed this information and agree to it by filling in the checkbox at the end of this document.
The Coaching Process
You have taken a very positive step by deciding to seek guidance. The outcome of your program depends largely on your willingness to engage in this process, which may, at times, result in considerable discomfort. Remember, there are no miracle cures. We promise to support you and do our very best to understand you and repeating patterns, as well as to help you clarify what it is that you want for yourself.
Cancellation Policy: the client is allowed to reschedule (1) one time during the course of this agreement without incurring a penalty, if cancellation is received within 24hours of the class. If cancellation is received less than 24hours before the scheduled class, the full session amount will be due. All schedule changes and cancellation notices must be submitted in writing via darian@mlsoe.com.
Confidentiality
The session content and all relevant materials to the client’s curriculum will be held confidential unless the client requests in writing to have all or portions of such content released to a specifically named person/persons. Limitations of such client held privilege of confidentiality exist and are itemized below:
  1. If a client conducts him/her self in a manner in which there is a substantial risk of incurring serious bodily harm.
  2. If a client threatens grave bodily harm or death to another person.
  • If a court of law issues a legitimate subpoena for information stated on the subpoena.
Occasionally we may need to consult with other professionals in their areas of expertise in order to provide the best treatment for you. Information about you may be shared in this context without using your name.
TERMS:
The Monica Lewis School of Etiquette (MLSOE) will be responsible for all print costs associated with this proposal for service. The client, The Monica Lewis School of Etiquette, will be responsible for selecting the venue (if not at MLSOE Houston Office), travel, hotel accommodations,  and attendance and any costs associated with thereof. The client will choose a date and time to host the class, date and time may not conflict with any other MLSOE event and session. An invoice will be provided for each session upon completion.
SERVICES AND PAYMENT. Consultant agrees to undertake and complete the Services as defined in this document by and on the schedule specified upon formal acceptance. As the only consideration regarding the subject matter of this Agreement, Client will pay Consultant by the terms laid out in this agreement.
  1. The client will be responsible for the prompt payment of all fees and costs incurred more than the  stated rate per course hour for 1:1 private courses; or posted course rate for semi-public and public courses and events.
  2. PAYMENT OF FEES: Absent other arrangements made in writing; payment is due immediately upon receipt of the billing statement. Charges not paid by the last business day of the billing month are considered delinquent and will bear interest at the rate of twenty percent (20%) per annum, or the maximum rate allowed by law, whichever is. In the event MLSOE must take legal action to collect Client’s account, Client expressly agrees to pay all collection costs, including reasonable attorney fees.
  3. Invoicing: MLSOE will submit an invoice that describes services and billable hours performed. All major credit cards are excepted. Corporate checks will be made payable to The Monica Lewis School of Etiquette.
Travel & Accommodations: For any services or events that require travel outside of the greater Houston metropolitan area or surrounding counties (Harris, Ft. Bend, Montgomery, or Galveston) travel and lodging accommodations are required; including flight, hotel, and per diem at $50 per day. Client shall book a flight and secure lodging for the consultant. Lodging and hotel accommodations must include a private room with controlled at access at a 3+ star hotel as determined by Hotels.com
Warranty. Consultant warrants that: (i) the Services will be performed in a professional and workmanlike manner and that none of such Services or any part of this Agreement is or will be inconsistent with any obligation Consultant may have to others; (ii) all work under this Agreement shall be Consultant’s original work and none of the Services or Inventions or any development, use, production, distribution or exploitation thereof will infringe, misappropriate or violate any intellectual property or another right of any person or entity (including, without limitation, Consultant); (iii) Consultant has the full right to provide the Company with the assignments and rights provided for herein; (iv) Consultant shall comply with all applicable laws and Company safety rules in the course of performing the Services and (v) if Consultant’s work requires a license, Consultant has obtained that license and the license is in full force and effect.
Termination. If either party materially breaches a material provision of this Agreement, the other party may terminate this Agreement upon five (5) days written notice unless the breach is cured within the notice period. Company also may terminate this Agreement at any time, with or without cause, upon five (5) days’ notice, but, if (and only if) without cause, Company shall upon termination pay Consultant all unpaid and undisputed amounts due for Services completed before notice of termination. This Agreement and any remedies for breach of this Agreement shall survive any termination or expiration. The company may communicate such obligations to any other (or potential) client or employer of Consultant.
Notice. All notices under this Agreement shall be in writing, and shall be deemed given when personally delivered, sent by confirmed telecopy or other electronic means, or three (3) days after being sent by prepaid certified or registered mail to the address of the party to be noticed as set forth herein or such other address as such party last provided to the other by written notice.
Miscellaneous. The failure of either party to enforce its rights under this Agreement at any time for any period shall not be construed as a waiver of such rights. No changes or modifications or waivers to this Agreement will be effective unless in writing and signed by both parties. If any provision of this Agreement shall be determined to be illegal or unenforceable, that provision will be limited or eliminated to the minimum extent necessary so that this Agreement shall otherwise remain in full force and effect and enforceable. This Agreement shall be governed by and construed by the laws of the state of Texas without regard to the conflicts of laws provisions thereof. Any legal action or proceeding relating to this Agreement shall be brought exclusively to relevant courts and each party consents to the jurisdiction thereof. In any action or proceeding to enforce rights under this Agreement, the prevailing party will be entitled to recover costs and attorneys’ fees. Headings herein are for convenience of reference only and shall in no way affect the interpretation of the Agreement. Any breach or threatened breach of Sections 2, 3 or 6 this Agreement will cause irreparable harm to the Company for which damages would not be an adequate remedy, and, therefore, the Company is entitled to injunctive relief with respect thereto (without the necessity of posting any bond) in addition to any other remedies. This Agreement constitutes the complete and exclusive agreement between the parties concerning its subject matter and supersedes all prior or contemporaneous agreements or understandings, written or oral, concerning the subject matter described herein.
Exclusions. This proposal does not include the costs associated with third-party vendors and service providers that may be required to deliver projects. These services may include but are not limited to Graphic Illustrators, Design, Photographers, Printing, Shipping, Drayage, Shipping, Equipment Rentals, or Trade/Exhibit costs if additional services are required MLSOE will submit all quotes and proposals to the client for approval before agreeing to services. MLSOE will not make any agreement, promises, or contracts with third parties without the express written consent from the client. If MLSOE hires or contracts a third-party vendor to deliver services, the client will be held harmless in this case.
